Repeat Prescription Request Form

Please use the form to request repeat medications for your pet.

Important: Our veterinarians are bound by Queensland’s veterinary legislation, including the Veterinary Surgeons Act 1936 (QLD) and the Health (Drugs and Poisons) Regulation 1996 (QLD). These regulations require us to keep accurate records of all medications dispensed and to ensure that prescriptions remain safe and appropriate for ongoing use.

As a result, the following conditions apply:

• Recent examination: Your pet must have been examined by one of our veterinarians within the past six months. If not, an appointment may be needed before more medication can be dispensed.

• Monitoring requirements: Some medications require more frequent check-ups or monitoring.

• Blood testing: Certain medications may require blood tests at specific intervals to ensure safety and effectiveness.

• Limited quantities: We can only prescribe or dispense limited quantities of medication at a time. 

Approval time: Each request must be reviewed and authorised by a veterinarian, so please allow up to two business days for preparation.
